Accountabilities
· The Lead Pharmacist, Care Pathways will lead a team of Clinical Pharmacists who will be designing, implementing, and evaluating clinical protocols that enhance patient care and optimize medication therapy management. This position requires a strong clinical background, excellent analytical skills, and a collaborative approach to working with healthcare teams.
ROLE AND RESPONSIBILITIES
- Clinical Team Leadership:
- Lead a team of pharmacists who design and develop clinical protocols for medical condition-specific medication management programs such as, but not limited to, Congestive Heart Failure, COPD, diabetes, hypertension, etc.
- Responsible for hiring, onboarding, and the development of a team of pharmacy personnel.
- Collaborate with multidisciplinary teams to ensure protocols align with guideline driven medication therapy, and other established clinical guidelines and best practices.
- Education and Training:
- Provide training and support to pharmacy staff and other healthcare professionals on the implementation of new protocols.
- Supervise the development of educational materials and resources for ongoing staff development.
- Quality Improvement:
- Partner with the Compliance and Quality teams to monitor and evaluate the effectiveness of clinical protocols and make recommendations for improvements.
- Participate in quality assurance initiatives to enhance medication safety and efficacy.
- Patient Care:
- Engage in direct patient care activities as needed, including medication reconciliation and therapeutic monitoring.
- Collaborate with healthcare providers to optimize pharmacotherapy for patients.
- Monitor and uphold adherence to evidence-based clinical guidelines and gold-standard treatment protocols across disease states.
- Regulatory Compliance:
- Ensure all protocols comply with regulatory and institutional policies and standards.
- Assist in preparing documentation for audits and regulatory reviews.
Minimum Qualifications and Requirements
- Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy school.
- Board of Pharmacy Specialties (BCPS) certification preferred
- PGY-1 residency or equivalent experience preferred
- Texas pharmacy licensure or eligibility.
- A minimum of 3-5 years of clinical pharmacy experience, with a focus on protocol development preferred.
- 3-5 years of management experience preferred
- Strong knowledge of pharmacotherapy, clinical guidelines, and quality improvement methodologies.
- Excellent analytical, organizational, and communication skills.
- Ability to work collaboratively in a multidisciplinary team environment.
- Proficiency in electronic health record (EHR) systems and pharmacy management software.
