POSITION OVERVIEW

We are seeking a licensed Clinical Pharmacist to support in a remote capacity. This role focuses on improving medication safety, optimizing therapeutic outcomes, and supporting interdisciplinary care teams. You will work standard weekday hours in a structured, outcome driven environment.

KEY RESPONSIBILITIES

• Review and verify medication orders for safety, accuracy, and clinical appropriateness
• Conduct comprehensive medication reviews and identify therapy gaps
• Provide evidence based recommendations to physicians and care teams
• Monitor chronic disease regimens and high risk medications
• Document clinical interventions in the electronic health record
• Support prior authorization and formulary management processes
• Provide patient counseling via phone or video
• Collaborate with physicians, nurse practitioners, nurses, and care coordinators
• Participate in quality improvement initiatives and medication safety programs
• Track and report clinical metrics tied to outcomes and cost management

TEAM STRUCTURE

You will work within a multidisciplinary team that includes physicians, nurse practitioners, registered nurses, case managers, and pharmacy support staff. The team culture emphasizes collaboration, accountability, and measurable clinical results. Daily communication occurs through virtual meetings and shared systems.

REQUIRED QUALIFICATIONS

• Doctor of Pharmacy degree
• Active Pharmacist license in good standing
• Minimum of 2 years clinical pharmacy experience
• Experience in ambulatory care, managed care, or population health
• Knowledge of chronic disease management guidelines
• Experience with electronic health records
• Strong written and verbal communication skills
• Ability to manage high volume clinical reviews with precision

PREFERRED QUALIFICATIONS

• Board Certification such as BCPS
• Experience with Medicare or Medicaid populations
• Prior remote or telehealth experience
• Experience with prior authorization workflows
• Background in value based care models

Doctoral degree required

LICENSURE

Active Pharmacist license required
