Job Details
All pharmacist positions at ExactCare Pharmacy are expected to deliver on quality standards that allow us to ensure proper medication profiles are managed, reviewed and verified. The Pharmacist is responsible for adhering to company policies and procedures and expected to help build team capabilities through leadership skills and follow up in a call center environment.
Essential Duties & Responsibilities:
Communication with healthcare professionals and making sure that the Dr. call line is answered.
Supervision and execution of incoming and outgoing prescription transfers
Perform DUR to identify medication therapy management eligible patients and identify clinical intervention per patient with follow up
Perform medication review and consultation
Ensure all legal, privacy and compliance requirements are met
Other duties as appropriate
Knowledge, Skills & Abilities:
Experience with Medication Therapy Management (MTM), comprehensive medication reviews and targeted interviews
Experiences with Pharmacist Consult Agreement (PCA), Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Transitional Care Management (TCM), Medications Reconciliation Post Discharge (MRPD), and Disease State Management.
Exceptional standards of customer care

Self-starter with little to no supervision needed
Requires the ability to actively engage members who are experiencing medication related issues and their prescribers.
Must have proficiency in pharmacology, therapeutics and knowledge of disease management programs for the purpose of supporting the clinical aspects of this job.
Requirements:
Bachelor’s degree in Pharmacy or Doctorate in Pharmacy (PharmD)
Active state of Ohio Pharmacist license in good standing
Completion of a post-graduate year 1 residency (PGY1) and/or Post-graduate year 2 residency (PGY2) or relevant experience preferred for Clinical Pharmacists
Bilingual in Spanish/English, a plus
Prior PCA Experiences
Certified Diabetes Educator (CDE), a plus
Clinical Judgement Expertise
Strong experience with Medication Therapy Management and comprehensive medication reviews
Prior interface with Electronic Medical Record; Athena, and ECW, a plus.
Prior interface with OutcomesMTM, Med Monitor, MTM Paths, and Docstation, a plus
Ability to work flexible schedule
Strong work ethic and ability to collaborate with multiple partners

Schedule: This is a full-time position with an expectation to work an average of 40 hours per week and be available outside of normal business hours to meet customer expectations on an ad-hoc basis. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
