Clinical Pharmacist - VytlOne Specialty-Remote

VytlOne is currently seeking an experienced Clinical Pharmacist to join our growing Specialty Pharmacy in Lubbock, TX. This is an excellent opportunity for a Pharmacist to make a positive impact in the lives of the patients we care for, with a devoted pharmacy operations team. The Clinical Pharmacist provides educational and consultant services in the field of pharmacology and drug therapy to patients and professional staff in accordance with all applicable laws, regulations, standards, policies and procedures.  Serves as a team leader for process improvement activities.  Provides educational development services to staff, serves medical staff to implement disease management guidelines, prepares outcome assessment reports, and helps clinic teams to optimize pharmacotherapy and prescription operational methods.  Supports clinics and pharmacies as assigned. Dispenses prescribed medications and ensures compliance with all applicable laws and regulations.

Responsibilities

 

ESSENTIAL FUNCTIONS (These duties must be performed with or without reasonable accommodation.  This document in no way states or implies that these are the only duties to be performed by the employee occupying this position):

 

  1. Interpret and fill drug orders accurately and timely
  2. Ensures the appropriateness of all medication orders, new prescriptions or refills, in reference to the client’s diagnosis, weight/body surface area, allergies, and drug interactions as evidenced by a review of the active medication profile, provider notes, and available pertinent laboratory information.
  3. Conduct/Review Drug Utilization Reviews to check for drug duplication, interactions or allergies.
  4. Review non-formulary drug requests using predetermined criteria.
  5. Review prior authorization requests using predetermined criteria.
  6. Maintain current knowledge of drug therapies and drug reactions and interactions by reading professional literature, attending seminars and through professional affiliations.
  7. Contact and confer with physicians regarding questions or irregularities with prescription practice whenever safety or established alternatives have not been utilized.
  8. Monitor drug therapy for contraindications, interactions, allergies and appropriateness.
  9. Documents / maintains initial patient assessments, reassessments and care management plans in the electronic patient management program, e.g., Asembia, CPR Plus.
  10. Provide drug information to meet the needs of medical and nursing staff members through direct contact or in-service programs.
  11. Provide patient information and counseling.
  12. Participate as an active member and facilitator in process improvement teams.
  13. Prepare provider newsletters or articles to keep providers updated. Provide individual and group training, in-services and other consultations to unit staff and providers to optimize therapy.
  14. Work with medical directors and staff in developing and carrying out various clinical initiatives.
  15. Generate and review standard monthly and quarterly reports, as required.
  16. Attend and participate in other programs, committees, meetings, and functions as required.
  17. Assist in the development of clinical, utilization management, ad hoc and production reports and programs.
  18. Assist in formulary, rebate analyses and pharmacoeconomic studies and initiatives.
  19. Perform retrospective drug utilization review: identify drug, physician, and patient utilization trends; make interventions as appropriate; monitor and document outcomes.
  20. Assist in the generation, maintenance, and distribution of formulary alternative lists, maintenance drug lists, and new drug to market lists.
  21. Work cooperatively with other staff members to enhance the team concept.
  22. Supervise pharmacy technicians in the routine performance of their duties.
  23. Participate in the development and implementation of the Performance Improvement Program.
  24. Assist in overseeing clinical procedures.
  25. Member of the Rare and Orphan Care Team
  26. Complete facility audits as assigned.
  27. Maintain regular attendance in accordance with established policies and procedures.
  28. Perform other job-related duties as assigned.
  29. Promptly report any allegations of impropriety to the Compliance Department.
  30. Comply with VytlOne’s Ethical Business Conduct policy and VytlOne’s Compliance Program.
  31. Remain free from exclusion under the OIG and SAMS Medicare/Medicaid lists.
  32. Complete required training, as assigned, within the established timeframes.
  33. Designated Person Only – Serve as the Designated Person by completing and maintaining all required training and certifications to ensure compliance with USP <797>, USP <800>, and all applicable company policies and procedures.

Qualifications

 

Experience: Residency or 2 years post-graduate practice experience in similar roles or equivalent demonstrated experience in a large organized health care system pharmacy strongly preferred but not required. Demonstrated self-motivation, strong leadership, team building abilities, and effective interpersonal communication.

 

EducationBS or Pharm. D. from an approved school of pharmacy.  Current unrestricted license to practice pharmacy in the state of employment is required. May be required to be licensed or become licensed in other states as needed.

  • Skill in the use of personal computers and related software applications.
  • Ability to develop and maintain recordkeeping systems and procedures.
  • Skill in examining and re-engineering clinical operations and procedures, implementing policy, and developing and implementing new strategies and procedures.
  • General knowledge of quality improvement principles and procedures.
