As our Clinical Pharmacist Ambulatory Services in the Anticoagulation clinic, you will work collaboratively with physicians, nurses, other healthcare providers, and patients to optimize drug therapy and outcomes using evidence-based standards of care.
Every day you will be responsible for providing effective, efficient and accurate ambulatory pharmaceutical care to achieve organizational, patient care, and quality measure goals and objectives, working within the scope of their license, performed in accordance with organizational protocols, internal guidelines/standards, and any applicable State regulatory requirements.

***This role is work from home within California, preferably in Northern California.
As a remote employee, we will provide you with the equipment needed to work from home, including a laptop, docking station, dual monitors, and accessories.
Required:
- Graduation from an accredited school of pharmacy
- Active CA Pharmacist License in good standing
- Current knowledge of evidence based clinical pharmacy practice and pharmacotherapies
- Knowledge of Health Plans such as Medicare, Medicaid, and commercial plans
- Demonstrates knowledge and skills necessary to provide care appropriate to all age groups of patients served and infection control, safety and risk management standards as relates to the ambulatory pharmacy setting.
Preferred:
- PharmD preferred
- Completion of three (3) years of practice experience with at least 50% of time spent in ambulatory care activities preferred
- Completion of a PGY2 Ambulatory Care Pharmacy residency preferred
- Completion of a PGY1 residency plus one (1) year of practice experience with at least 50% of time spent in ambulatory care activities preferred
- Anticoagulation experience preferred
