This is temporary MTM Pharmacist remote position for a duration of 3-4 months only.

Job Overview:
The Clinical MTM Pharmacist works remotely and evaluates and optimizes patients’ 
medication regimens via phone or virtual platforms, documents interventions, and 
collaborates with prescribers to improve safety, adherence, and quality outcomes. The 
role focuses on comprehensive medication reviews, targeted interventions, and closing 
quality gaps (e.g., Medicare Star measures) without routine dispensing responsibilities
This position requires clinical excellence, autonomy, strong collaboration with 
multidisciplinary teams, and a commitment to patient-centered, community-based care.


Key Responsibilities
• Perform comprehensive and targeted medication reviews (CMRs/TMRs) to assess 
indication, effectiveness, safety, and adherence for all chronic medications.
• Conduct telephonic or video MTM consultations, educate patients, identify 
medication-related problems, and develop and communicate individualized care 
plans.
• Coordinate with prescribers and care teams to recommend therapy changes, 
resolve drug therapy problems, and support guideline-concordant prescribing.
• Document encounters, care plans, and interventions in the MTM or clinical platform 
in a timely, accurate, and compliant manner.
• Support quality and value-based initiatives (e.g., Medicare Part D MTM, adherence, 
high-risk meds, statin use in diabetes, opioid safety) through proactive outreach 
and follow-up.
• Review pharmacy and medical claims data to identify MTM-eligible members and 
clinical intervention opportunities
• Assist in developing patient and provider educational materials and contribute to 
process improvement of MTM workflows.
• Maintain current knowledge of MTM program requirements, CMS guidance, and 
health plan or PBM policies impacting MTM operations.
• Perform additional duties as assigned and within scope of pharmacy practice


Requirements
• Active, unrestricted California Pharmacist license in good standing
• Strong clinical judgment in chronic disease pharmacotherapy (e.g., diabetes, 
hypertension, HF, COPD, behavioral health).
• Excellent telephonic communication skills, motivational interviewing, and health 
literacy–sensitive patient education.
• High attention to detail with efficient documentation and time management in a 
remote environment.
• Comfort with multiple technology platforms (EHR/MTM platforms, virtual call 
systems, data dashboards) and basic data interpretation.
• Ability to work independently with minimal direct supervision
• Consistent application of standard precautions and infection prevention and 
control practices in alignment with CDC and organizational guidelines
• Commitment to patient-centered, equitable, and compassionate care

Preferred Qualifications
• Prior MTM, ambulatory care, managed care, or clinical pharmacy experience 
strongly preferred.
• Current malpractice insurance maintained at all times
